Mechanism

Ramipril inhibits the conversion of angiotensin I to angiotensin II, a potent vasoconstrictor. This leads to vasodilation, reduced aldosterone secretion, decreased sodium retention, and ultimately lowers blood pressure while protecting the heart and kidneys.

Pharmacokinetics

  • Absorption: Rapidly absorbed after oral administration
  • Bioavailability: ~50–60%
  • Distribution: Converted to active metabolite (ramiprilat)
  • Metabolism: Hepatic metabolism to ramiprilat
  • Elimination: Excreted via urine and feces
  • Half-life: 13–17 hours (ramiprilat)

Side Effects

  • Dry cough
  • Dizziness
  • Hypotension
  • Hyperkalemia
  • Fatigue

Contraindication

  • History of angioedema
  • Pregnancy
  • Severe renal artery stenosis
  • Hypersensitivity to ACE inhibitors

Interaction

  • Potassium supplements & potassium-sparing diuretics
  • NSAIDs (may reduce effect)
  • Diuretics (increase hypotension risk)
  • Lithium (increased toxicity risk)

Warning and Precautions

  • Monitor renal function and potassium levels
  • Use cautiously in renal impairment
  • Avoid during pregnancy
  • Start with low dose in elderly patients
